HOW TO ENTER THE
SUPER
SERIES
WINTER
NATIONAL CHAMPIONSHIPS
With the
exception
of the 9U National Division, 15U, 16U, and 17-18U divisions, if
there is a Winter National qualifying tournament in your state,
teams from your state
must have qualified
in a Super Series Fall/Winter qualifying tournament. The 9U
National, 15U, 16U, and 17-18U age divisions do not require
qualification for the Winter Nationals.
IF
THERE IS NO FALL/WINTER QUALIFIER IN YOUR STATE, YOUR TEAM MAY
RECEIVE AN
AT-LARGE
ENTRY IN THE NATIONAL DIVISION.
Teams
desiring AT-LARGE entry should remit a completed entry form and
entry fee to the designated tournament director for your age
group.
There is no 8 year old competition in the Winter National Championships. 8 year old competition is available only in
the Summer National Championships.

NOT to be sent to the Super Series national office.
While hundreds of teams will have
qualified for each National Championship event, entries are
limited. Some age/divisions may be full PRIOR to our entry
deadline. Don't wait until
the last minute - your age division may already be full.

Copy of
Super Series Team Roster / Registration
Make sure
you have 2 printed, current copies of your roster/registration.
Any player additions or deletions must be made through the
ONLINE Roster Control Panel.
NO HANDWRITTEN changes or
entries will be accepted on any roster at team check-in.
Be sure to check all spellings of players' names. The
spelling
on your roster MUST MATCH the spelling on the birth
certificate.
Copy of all
player State Birth Certificates
You will have to turn in
a complete set of birth certificate copies at
check-in.
Original Proof of Date of Birth
Coaches will also have
to show an original state or federal
document attesting to the date of birth for each player
(Hospital
certificates are not acceptable)
For ages 16's and above
- a state or federal photo ID (such as a state driver's license
is acceptable)
A U.S. Passport is an acceptable
replacement for original birth certificates in all ages.
Copy of your
National Entry Form
In
the event Player Identity is Questioned:
Super
Series wishes to ensure fairness. If the identity of a
player is questioned, we will ask to see the original Social
Security card for any player 15 years of age or younger.
For players 16 and older, a driver's license with photo will
be required.
SUPER
SERIES TOURNAMENTS
ENTRY REFUND POLICIES:
There
are no refunds for withdrawal
within 30 days of the start of an event.
Teams withdrawing from an event
more than 30 days in advance
shall receive a full refund of their entry fee,
less a $25.00 handling fee.
